有料アプリ契約(App Store) 02 納税フォームの記入

有料アプリ契約を行うためには、納税フォームに記入しなければなりません。
記入の仕方自体は、先人の記事を参考に進めることができたのですが、一つ問題が発生しました。
U.S. Certificate of Foreign Status of Beneficial Ownerと U.S. Substitute Form W-8BEN-Eの法人形態の選択肢に個人事業主（Individual/Sole Proprietorがないのです。
Developer Support へ連絡したところ、以下の回答。
納税フォームの問題については、専門の Finance チームに直接お問い合わせ下さい。
「 Contact Us About Financial Information」(ファイナンス情報に関するお問い合わせ) にアクセスして、ご質問を送信してください。支払いに関する質問には、トランザクション ID または Consolidated Credit Identifier (CII) (統合クレジット識別情報) を添えてください。フォローアップ番号が記載された自動送信メールが届きます。 Finance チームへのご依頼は英語でのみ受け付けておりますので、あらかじめご了承ください。
だそうで、英語で問い合わせ、一週間待ち、回答。
You are required to complete the US tax form W8BEN-E online. Unfortunately, this means that you will need to complete the form as it appears now, as best as possible, and select any other type of beneficial owner. **Please note: The Company name must appear in line 7, REFERENCE NUMBER.
Once completed and your paid agreement is ACTIVE, you will need to download a new W8BEN from the IRS website, then submit the updated digital copy to us.
自分で、IRS websiteからW8BENのフォーマットを探し、アップロードしました。
なんとか納税フォームのステータスは有効になり、有料アプリ契約も有効に。
これで、有料版「Title Call Pro」がApp Storeに登場しました。
